JAVA RIDING SCHOOL RELEASE FORM
Only riders are allowed to attend the JAVA Riding School event, no spectators. Minors must be accompanied by a parent or guardian. Parent or guardian must maintain a minimal presence to minimize disruption or the training. A $100.00 non refundable deposit is due before a date may be scheduled with the balance due immediately after the event has ended. JAVA Riding School has a no refund policy. Cancellations by a student must be received and confirmed by JAVA Riding School will either be rescheduled or refunded if necessary.
I, (please print your full name) ____________________________hereby release Jocelin and all JAVA Riding School employees and personnel, contractors, friends, agents and assignees (hereinafter referred to as "Released Parties") from any and all injuries suffered by myself, and anyone who accompanies me in the JAVA Riding School event and traveling to and from same, or while participating in any JAVA school activities. I am aware of the hazards inherent in motorcycle riding. Furthermore, I understand that motorcycle riding and all JAVA school activities involve certain risks such as impact with other students or instructors, natural or man made obstacles. I understand that crashing is necessary for training and it involves the same risks. I understand that if I have brought a motorcycle, gear or property of my own to use during the school it may be damaged and possibly ruined. I understand that the 'Released Parties" can not be held liable or responsible in any way for injury, death or other damages to me or my equipment that may occur as a result of my participation in the JAVA Riding School, or as a result of negligence of any party, including the "Released Parties" whether passive or active. I authorize Jocelin and the JAVA Riding School staff to act for me according to their best judgment in any emergency requiring medical attention.
